Manage all aspects of transfer from hospital to home/assisted living, and other environments to assure a smooth transition
Explore community events/hobbies of Interest
Represent and advocate for family at hospital discharge planning meetings
Organize and manage multiple caregivers
Prepare for sale of home
Arrange and manage relocation out of state
Living Memories Program- to improve quality of life for dementia clients
Advocate and initiate home-care services, both through insurance and privately paid services
Accompany to provider appointment to assure client is understanding their condition, choices and options
Prepare information on available housing options, set-up tours, (in person and virtual) arrange for moving belongings, complete necessary paperwork and accompany to new living environment.

Every plan is individualized, if you would like more or another frequency of home visits we can accommodate those needs. Regardless of visiting category the following is included:
Visitation and evaluation in home setting
Updates via email or phone, (standard 30 minutes), per month, or anytime there is a significant change, to one agreed upon family representative
Set-up, coordinate, and monitor all other homecare services. Utilizing those covered by insurance first, then private services as needed.
Provide education and guidance one Advanced Care Planning
Manage medical appointments, set-up transportation, and accompany as needed, up to once per month.
